About Fees and Insurance Billing
Most people desire a personal physician who gets to know them well, takes time to listen and care, and prescribes tests and treatments and medications that are in the best interest of the patient. I am that kind of personal physician.
Because I practice medicine in this manner, I am not a "contracted participating preferred provider" in any medical insurance plan, because insurance plans often limit care based on the limits of the insurance benefits policy.
Most insurance companies will cover my services as an "out-of-network" doctor. My fee is due in full at the time of service, and my staff will provide you with a special "CMS-1500 Claim Form" receipt as you leave our office which can be sent as-is to your insurance company. Your insurance company will pay your benefits directly to you. You can still use your insurance for the laboratories, specialists and pharmacies in your plan. You can check with your insurance agent in advance about your benefits. Eliminating the insurance payment piece, also allows me to keep staffing costs down and in return, keep my fees reasonable.
If you are unable to pay in full at the time of your visit, please feel free to discuss a payment plan with us.
